Cilostazol falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the risk of bleeding.
In vitro research and animal research shows that horny goat weed can inhibit platelet aggregation and thrombus formation. This effect has not been reported in humans.

Here's the thing to keep in mind: cilostazol (Pletal) already works to keep your platelets from clumping together, which is how it improves blood flow to your legs. Horny goat weed may do something similar. In test tube and animal studies, it appears to slow platelet clumping and clot formation. Stack that on top of cilostazol and, at least in theory, you could be a bit more prone to bleeding or bruising.

I want to be honest about the evidence here, though. This bleeding effect has only shown up in lab and animal research, never in real people, so it's more of a theoretical caution than a proven problem. Still, watch for easy bruising, nosebleeds, or bleeding gums, and chat with your pharmacist or doctor before combining the two.

Cilostazol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the connection worth knowing about. Your body relies on a liver enzyme called CYP3A4 to break down and clear cilostazol. In test-tube studies, horny goat weed seems to slow that same enzyme down. If that happened in real life, cilostazol could clear more slowly and build up in your blood, which may mean stronger effects and a higher chance of side effects like headache, dizziness, a fast or pounding heartbeat, diarrhea, or easier bruising.

The honest part: this has only been seen in lab dishes, not in actual people, so it's theoretical and far from certain. Still, because cilostazol already affects blood flow and clotting, I'd play it safe. Check with your pharmacist or doctor before pairing these, rather than starting or stopping anything on your own.
