Why this applies to Mexiletine
Mexiletine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Horny Goat Weed interacts with.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for mexiletine: your liver relies heavily on an enzyme called CYP1A2 to break this medicine down and clear it from your body. In lab (test tube) studies, horny goat weed appears to slow that same enzyme down. If that held true in people, mexiletine could build up to higher levels than intended.
Why that matters: mexiletine is a heart rhythm drug with a fairly narrow safe range, so even modest increases could mean more side effects like nausea, dizziness, tremor, blurred vision, or in rarer cases new rhythm problems. The honest caveat is that this has only been seen in the lab, never confirmed in humans, so it stays theoretical. Still, given what mexiletine does, I'd check with your pharmacist or doctor before combining them.

Mexiletine hydrochloride capsules treat documented ventricular arrhythmias that your doctor judges to be life-threatening. One example is sustained ventricular tachycardia (a fast, dangerous heartbeat from the lower heart chambers).
It is generally not recommended for milder rhythm problems. It should also be avoided for asymptomatic ventricular premature contractions (extra heartbeats that cause no symptoms).
