Modafinil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Your body clears modafinil partly through a liver enzyme called CYP3A4. In test-tube studies, horny goat weed seems to slow that same enzyme down. If that happened in people, modafinil could hang around a bit longer than usual, nudging up its effects and side effects. With modafinil that might mean things like trouble sleeping, headache, jitteriness, a faster heartbeat, or a queasy stomach.

Here's the reassuring part: this is only a theoretical concern based on lab work, not anything actually seen in real patients. So I wouldn't panic, but I also wouldn't ignore it. Before combining the two, have a quick chat with your pharmacist or doctor so they can weigh it for your situation.

What Is Modafinil Used For?

Brand names: Provigil, Sparlon, Alertec

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).

In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
