Horny Goat Weed and Paracetamol

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Why this applies to Paracetamol

Paracetamol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Tylenol-type pain reliever (paracetamol/acetaminophen) and horny goat weed. In a test tube, this herb slowed down an enzyme called CYP1A2, which is one of the pathways your liver uses to process certain medicines. The theory is that if that enzyme is blocked, a little more paracetamol could hang around in your body, which might nudge up its effects or side effects.

The honest part: this has only been seen in lab dishes, never in real people. Paracetamol leans on several liver pathways, not just CYP1A2, so the concern here is mild and mostly theoretical. Still, if you take paracetamol often or have liver concerns, it's worth a quick chat with your pharmacist before adding this supplement. Don't change anything on your own.

From our pharmacist’s overview on the Paracetamol page

“Nothing on this list concerns me more than alcohol for people who take paracetamol (Panadol) regularly. Beer and wine both carry major ratings because paracetamol is processed by the liver, and drinking on top of it raises the odds of genuine liver injury.”

  • General 'vitality' or performance

Horny goat weed (Epimedium) is a traditional Chinese herb most often marketed for low libido and erectile problems, but solid human evidence for these uses is lacking. While short-term use seems generally tolerated by some people, quality and safety are not well studied, so talk to your pharmacist or doctor before trying it—especially if you have heart problems or take other medicines.
