Pomalidomide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.

Here's the gist for Pomalyst (pomalidomide). Your body partly clears this medicine using a liver enzyme called CYP1A2. In a test-tube study, horny goat weed slowed that same enzyme down. If that happened in real life, pomalidomide could build up a bit and you might see stronger effects, like more low blood counts (fatigue, infections, easy bruising or bleeding), or more tiredness. Pomalidomide is active on its own, not a prodrug, so blocking the enzyme would tend to raise levels, not lower them.

That said, this is only laboratory data and has never been shown in people, so it may not matter at all. Because this is a serious cancer medicine, please run any supplement past your oncology team or pharmacist before adding it.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the gist for Pomalyst (pomalidomide). Your body uses an enzyme called CYP3A4 to help clear this medicine. In the test tube, horny goat weed appears to slow that enzyme down. If that same effect happened in people, less of the drug would be broken down, so more could build up in your system. That could mean stronger effects and a higher chance of pomalidomide side effects like low blood counts, fatigue, infections, or blood clots.

Now the reassuring part: this is only lab-dish data and has never been shown in actual people, so the real-world risk is uncertain. Still, because pomalidomide is a serious cancer medicine, please don't add this supplement without first checking with your oncologist or pharmacist.

Horny goat weed (Epimedium) is a traditional Chinese herb most often marketed for low libido and erectile problems, but solid human evidence for these uses is lacking. About Pomalidomide

What Is Pomalidomide Used For?

Brand name: Pomalyst

Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.

Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
