Ramelteon falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.

Here's the gist for Rozerem (ramelteon): your liver leans on an enzyme called CYP1A2 to clear it from your body. In a lab dish, horny goat weed slowed that enzyme down. If the same thing happened in people, ramelteon could hang around longer and feel stronger than usual. With a sleep aid, that mostly means next-morning grogginess, drowsiness, dizziness, or feeling a bit foggy.

The good news is that this is only a theoretical concern. It has been seen in test tubes, not in actual patients, so we really don't know if it matters in real life. Still, if you take Rozerem and want to try horny goat weed, run it past your pharmacist or doctor first so they can keep an eye on you.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the gist with Rozerem (ramelteon): your liver clears it using an enzyme called CYP3A4. In a test-tube setting, horny goat weed seems to slow that same enzyme down. If that happened in your body, less of the drug would get broken down, so more could hang around. With ramelteon, that usually shows up as feeling extra drowsy, groggy, dizzy, or hungover the next morning.

I want to be honest about the strength of this, though. This is only lab data, and it has not been seen in real people, so it stays in the theoretical bucket for now. Still, since both are working on sleep, it's worth a quick chat with your pharmacist or doctor before combining them.

Ramelteon tablets — sold as the brand Rozerem and as generic ramelteon — are used to treat insomnia when the main problem is trouble falling asleep at the start of the night. It is not indicated for difficulty staying asleep.

Clinical studies supporting its effectiveness ran up to six months in both adults and elderly patients.
