Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Simeprevir Sodium
Simeprevir Sodium falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: your liver uses an enzyme called CYP3A4 to help clear simeprevir (Olysio) out of your system. In test-tube studies, horny goat weed appears to slow that same enzyme down. If that happened in your body, simeprevir could build up a bit higher than expected, which might mean stronger effects and more side effects like nausea, itching, rash, or sensitivity to sunlight.
The important catch: this has only been seen in lab dishes, not in real people. So we honestly don't know if it matters at the doses folks actually take. Still, hepatitis C treatment is a big deal, so I'd loop in your pharmacist or doctor before adding this herb rather than guessing on your own.

“Hepatitis C treatment with simeprevir (Olysio) runs for months, and steady drug levels decide whether the virus gets cleared, so the 160 supplements on this page deserve real scrutiny. Only three carry a major rating, but they are common ones: grapefruit and St.”

- General 'vitality' or performance
Horny goat weed (Epimedium) is a traditional Chinese herb most often marketed for low libido and erectile problems, but solid human evidence for these uses is lacking. While short-term use seems generally tolerated by some people, quality and safety are not well studied, so talk to your pharmacist or doctor before trying it—especially if you have heart problems or take other medicines.
About Simeprevir
What Is Simeprevir Used For?
Brand name: Olysio
-
Simeprevir is used along with ribavirin (Copegus, Rebetol) and peginterferon alfa (Pegasys) to treat chronic hepatitis C (an ongoing viral infection that damages the liver). Simeprevir is in a class of medications called protease inhibitors. It works by decreasing the amount of hepatitis C virus (HCV) in the body. Simeprevir may not prevent the spread of hepatitis C to other people.
