Sonidegib Phosphate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the gist for Odomzo (sonidegib). Your body clears this cancer medicine using a liver enzyme called CYP3A4. In test-tube studies, horny goat weed seems to slow that same enzyme down. If that were to happen in people, sonidegib could build up in your system more than expected, which may mean stronger side effects like muscle cramps or muscle pain, fatigue, nausea, hair loss, or changes in taste. Sonidegib is the active drug itself, not a prodrug, so more in the bloodstream means more effect.

The important catch: this is only laboratory data, and it has never actually been shown in humans. So I wouldn't panic. Still, because sonidegib is a serious medication, please run any supplement past your oncology team or pharmacist before combining them.
