Why this applies to Tacrine

Tacrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.

Here's the connection: your body relies on a liver enzyme called CYP1A2 to break down tacrine and clear it out. In lab studies (test tube only, not in real people), horny goat weed seems to slow this enzyme down. If that happened in your body, tacrine could build up higher than expected and hit you harder.

Tacrine isn't a prodrug, so more of it in your system means stronger effects and more side effects, things like nausea, stomach upset, diarrhea, dizziness, and importantly liver irritation, which tacrine is already known for. The honest truth is this is theoretical and hasn't been shown in humans, so I wouldn't panic. Still, please run it by your pharmacist or doctor before combining them.
