Horny Goat Weed and Tizanidine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Why this applies to Tizanidine

Tizanidine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the situation with these two. Your body relies heavily on an enzyme called CYP1A2 to clear tizanidine out of your system. In lab studies (test-tube only, not in real people yet), horny goat weed has been shown to slow that same enzyme down. If it did the same thing inside you, your tizanidine could build up and hit harder than expected.

For tizanidine that's worth respecting, because more drug usually means more drowsiness, dizziness, very low blood pressure, and feeling weak or lightheaded, especially when you stand up. The good news: this is a theoretical concern based only on lab work, so we don't know it actually happens in humans. Still, since tizanidine is sensitive to CYP1A2 changes, I'd check with your pharmacist or doctor before combining them.

About Horny Goat Weed

What is Horny Goat Weed?

Horny Goat Weed
  • Erectile dysfunction
  • Low libido
  • Fatigue and low energy
  • Bone health
  • Joint and menopausal symptoms
  • General 'vitality' or performance

Horny goat weed (Epimedium) is a traditional Chinese herb most often marketed for low libido and erectile problems, but solid human evidence for these uses is lacking. While short-term use seems generally tolerated by some people, quality and safety are not well studied, so talk to your pharmacist or doctor before trying it—especially if you have heart problems or take other medicines.

About Tizanidine

What Is Tizanidine Used For?

Brand names: Zanaflex, Ontralfy

Tizanidine — available as tablets, capsules, and as the oral solution Ontralfy — is used to manage spasticity (abnormal muscle stiffness or spasms). Because it works for a relatively short window of time, it is typically scheduled around the activities when spasm relief matters most.

Ontralfy (oral solution) and Zanaflex capsules are specifically indicated for the treatment of spasticity in adults. Other tizanidine tablets and capsules are indicated for the management of spasticity more broadly.
