Tramadol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the gist for tramadol. Your body uses an enzyme called CYP3A4 (along with another one, CYP2D6) to process this medicine. In test-tube studies, horny goat weed seems to slow CYP3A4 down a bit. If that happened in real life, tramadol could linger longer and hit a little harder, meaning more chance of drowsiness, dizziness, nausea, constipation, or feeling foggy.

The important part: this is only lab evidence, and it has not been shown in actual people, so I wouldn't lose sleep over it. Still, tramadol already carries seizure and serotonin risks, so it's worth a quick chat with your pharmacist or doctor before combining the two. Don't change your tramadol on your own.
