Treprostinil (IV) falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Horny Goat Weed interacts with.
In vitro research and animal research shows that horny goat weed can inhibit platelet aggregation and thrombus formation. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Treprostinil (Remodulin): besides relaxing blood vessels and lowering pulmonary pressures, it also slows down your platelets, the cells that help your blood clot. Horny goat weed may do something similar. In lab and animal studies, its compounds appear to keep platelets from clumping together. Stack those two effects and, in theory, you could bruise or bleed a bit more easily.
Now, the honest part: this is only seen in test tubes and animals, never reported in actual people, so the concern is mostly on paper. Still, with an IV medicine as serious as treprostinil, I'd loop in your doctor or pharmacist before adding this herb rather than guess. Watch for easy bruising, nosebleeds, or bleeding gums.

What Is Treprostinil Injection Used For?
Brand name: Remodulin
Treprostinil injection is used to treat pulmonary arterial hypertension (PAH, WHO Group 1) — high blood pressure in the arteries of the lungs — to reduce symptoms during exercise. It has been studied in patients with moderate to severe PAH symptoms (NYHA Class II–IV) caused by idiopathic or heritable PAH, PAH linked to congenital heart defects, or PAH associated with connective tissue diseases.
It is also used for patients with PAH who need to transition off epoprostenol (another PAH medicine), to slow the rate of disease worsening during that switch. The benefits and risks of transitioning should be carefully weighed by the prescriber.
