Voxelotor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the situation with these two together. Your body relies on a liver enzyme called CYP3A4 to clear voxelotor (Oxbryta) out of your system. In lab tests, horny goat weed appears to slow that same enzyme down. If that were to happen in real life, voxelotor could build up a bit higher than expected, which might mean more of its usual side effects, things like headache, nausea, diarrhea, belly pain, or a rash.

The good news: this is only a theoretical concern based on test tube data, and it has not actually been shown to happen in people. So I wouldn't panic. Still, since voxelotor is an important medicine for your sickle cell disease, it's worth a quick chat with your pharmacist or doctor before adding this herb.

Brand name: Oxbryta

Oxbryta (voxelotor) is approved to treat sickle cell disease (SCD) in adults and in children as young as 4 years old. It works by targeting the underlying problem in sickle cell disease — the abnormal hemoglobin that causes red blood cells to take on a rigid, sickle shape.

This approval was granted under the FDA's accelerated approval pathway, based on the drug's ability to raise hemoglobin levels. Continued approval may depend on results from confirmatory clinical trials showing broader clinical benefit.
