Here's the wrinkle with this combo. The codeine in your Phrenilin #3 is actually a prodrug, which means it has to be switched on by a liver enzyme called CYP2D6 before it gives you much pain relief. Grape seed extract in these chews might gently slow that same enzyme. So in theory, less codeine would get turned into its active form, and you could notice weaker pain relief rather than more side effects.

The good news: this is based only on lab studies, the evidence is weak, and it has never actually been reported in real people. The amount in two chews is small, so for most folks it's unlikely to matter. Still, if your headache pain isn't controlled the way it used to be, mention it to your pharmacist or doctor before changing anything.

Here's the piece that matters for your Phrenilin with Codeine combo: the acetaminophen part is partly handled by a liver enzyme called CYP2E1. The grape seed extract in these chews has shown, in lab dishes only, a mild ability to slow that enzyme down. In theory, slower breakdown could let a little more acetaminophen linger, and acetaminophen at very high levels is hard on the liver.

Honestly though, this is a minor and unlikely concern. There are no real human cases of it happening, and the codeine and butalbital pieces aren't really affected here. The bigger thing to watch is that beet-based chews can lower blood pressure, while this medicine can already make you drowsy or lightheaded. Just give your pharmacist a heads-up before pairing them.
