Here's the wrinkle with this combo. The codeine in Mersyndol is actually a prodrug, meaning it doesn't do much until your body converts it into morphine using an enzyme called CYP2D6. Grape seed extract in these chews might gently slow that same enzyme down, at least in lab studies. So instead of building codeine up, slowing CYP2D6 would likely mean less morphine is made, and you might get a bit less pain relief than expected.
Honestly though, this is very theoretical. There are no real human reports of it happening, the amount of grape seed in these chews is small, and the effect rated as minor and unlikely. I wouldn't lose sleep over it. If your pain medicine ever seems weaker than usual, just mention it to your pharmacist or doctor.

Here's the gist of it. The grape seed extract in SuperBeets Heart Chews may, at least in lab dishes, gently slow down an enzyme called CYP2E1. The acetaminophen part of Mersyndol relies partly on that enzyme. In theory, if it's slowed, a bit more acetaminophen could linger in your system, which mostly matters for liver stress at high or frequent doses rather than for everyday side effects.
Honestly though, this one is very weak. It's based only on test-tube work, it's rated minor and unlikely, and it has never been reported in real people. The codeine and doxylamine pieces aren't really driven by this enzyme. I wouldn't lose sleep over it, but since acetaminophen and your liver deserve respect, check with your pharmacist before pairing them.

Honestly, though, this hasn't been seen in real people, the evidence is weak, and the concern is rated minor and unlikely. The codeine and doxylamine parts aren't really driven by this enzyme. I wouldn't lose sleep over it, but since acetaminophen and liver health do go hand in hand, it's worth a quick chat with your pharmacist, especially if you drink alcohol or use acetaminophen often.
