In practice this is a minor effect that tends to be small and short-lived, but it could nudge you toward feeling dizzy, lightheaded, or a bit tired, especially when you stand up quickly. Bisoprolol is not a prodrug, so this isn't about how it's broken down, it's just two blood-pressure-lowering effects adding up. The grape seed extract in each serving might gently slow down a liver enzyme called CYP2D6, which is one pathway your body uses to clear bisoprolol. In theory, if that enzyme is blocked a bit, bisoprolol could hang around a little longer and build up slightly. With a beta blocker like this, that could mean a touch more of its usual effects, things like a slower heart rate, lower blood pressure, lightheadedness, or feeling more tired than usual.
That said, I don't want to overstate this. This is based only on lab studies, there are no real reports of it happening in people, and it's rated as minor and unlikely. Bisoprolol also doesn't lean heavily on CYP2D6 to begin with. Still, it's worth mentioning to your pharmacist or doctor, especially if your blood pressure already runs low.
