Here's the wrinkle with tramadol. It's actually a prodrug, meaning your liver uses an enzyme called CYP2D6 to turn it into its stronger active form (O-desmethyltramadol), which does most of the pain relief. The grape seed extract in these chews might gently slow that same enzyme in lab tests. So in theory, you could convert a little less tramadol into its active form, which might mean slightly weaker pain control rather than more side effects.
That said, this is very theoretical. It comes only from test-tube data, there are no real human reports, and it's rated minor and unlikely. The small amounts here probably won't change much. Still, if your pain isn't well controlled, mention these chews to your pharmacist or doctor before changing anything.

Tramadol's effects also depend heavily on another enzyme (CYP2D6), so CYP3A4 is just part of the picture.
