Huperzine A and Amitriptyline, Perphenazine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICHOLINERGIC DRUGS

Why this applies to Amitriptyline, Perphenazine

Amitriptyline, Perphenazine falls under ANTICHOLINERGIC DRUGS — the group Huperzine A interacts with.

Theoretically, huperzine A might decrease the effects of anticholinergic drugs.
Huperzine A has acetylcholinesterase (AChE) inhibiting effects. In animal models, huperzine A reversed cognitive deficits induced by scopolamine, an anticholinergic drug.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version. Triavil combines amitriptyline (an antidepressant) with perphenazine (an antipsychotic), and both of these drugs have anticholinergic activity, meaning they block the brain chemical acetylcholine. Huperzine A works in the opposite direction by raising acetylcholine levels. So in theory, the two could partly cancel each other out, making your Triavil a little less effective for things like mood or sleep.

I want to be honest about the evidence here: this is only suggested by lab and animal studies, not real-world human reports, so it may not amount to much in practice. Still, since both medicines are active and Huperzine A isn't well standardized, it's worth running it past your pharmacist or doctor before combining them.

  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Amitriptyline, Perphenazine page

“Alcohol is the risk I'd flag first for anyone taking amitriptyline with perphenazine (Triavil, Etrafon). Beer and wine both carry major ratings here, and 78 of the 311 ingredients on file deepen the drowsiness this antidepressant and antipsychotic combination already causes, sometimes to the point of slowed breathing.”

About Huperzine A

What is Huperzine A?

Huperzine A
  • Memory and cognitive support
  • Alzheimer's disease and dementia
  • Age-related memory decline
  • Focus and mental performance
  • Myasthenia gravis (traditional/older use)

Huperzine A is a purified compound from a Chinese clubmoss that acts like a mild cholinesterase inhibitor, similar in mechanism to some prescription Alzheimer's drugs. Some small studies suggest it may modestly help memory in people with dementia, but the evidence is limited and the quality is uncertain. Because it is biologically active and not well standardized, talk with a pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.
