Here's the concern with this pair: Hydroxychloroquine (Plaquenil) is one of those medicines that can stretch out the heart's electrical reset time, something doctors call QT prolongation. Iboga, through its active compound ibogaine, has been tied to the same effect in a number of case reports. Stack the two together and the worry is an additive push that could trigger a dangerous, irregular heartbeat with symptoms like palpitations, dizziness, fainting, or worse.

I'll be honest, the evidence here is thin and largely theoretical, based on scattered reports rather than solid studies. But ibogaine is risky and illegal in the U.S. on its own. Please don't combine these without talking to your doctor or pharmacist first, who may want to check your heart rhythm.

Hydroxychloroquine also falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS.

Theoretically, taking iboga with CYP2D6 inhibitors might increase the levels and clinical effects of ibogaine.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.

Here's the key thing to understand about this pairing: it's the Iboga that gets affected, not your Plaquenil. Hydroxychloroquine acts as a mild blocker of an enzyme called CYP2D6, and your body uses that same enzyme to break down ibogaine, the active compound in Iboga. So if the two are taken together, ibogaine and its byproduct can build up to higher levels than expected. In one study, a similar blocker roughly doubled ibogaine exposure.

That matters because Iboga is already risky. Higher levels can mean a greater chance of dangerous heart rhythm changes and intense effects on the nervous system. Ibogaine is illegal in the U.S. and not something I'd recommend using. If you're taking hydroxychloroquine and considering anything like this, please talk it over with your doctor or pharmacist first.

What Is Hydroxychloroquine Used For?

Brand name: Plaquenil

Hydroxychloroquine tablets are used to treat uncomplicated malaria and to help prevent malaria in areas where the parasite has not developed resistance to it — in both adults and children. It is also used in adults to treat rheumatoid arthritis, systemic lupus erythematosus (SLE), and chronic discoid lupus erythematosus.

There are important limits: it is not recommended for complicated or drug-resistant malaria, malaria acquired in regions where resistance is known, or to prevent malaria relapses caused by P. vivax or P. ovale. Its use for the autoimmune conditions (rheumatoid arthritis and lupus) is approved for adults only.
