Trimethoprim falls under QT INTERVAL-PROLONGING DRUGS — the group Iboga interacts with.

Theoretically, iboga may have an additive effect with drugs that prolong the QT interval.
Numerous case reports suggest that ibogaine, a constituent in iboga, can cause QT interval prolongation.

Here's the concern with these two together. Trimethoprim, the antibiotic in Proloprim and Trimpex, can occasionally nudge the heart's electrical timing, what doctors call the QT interval. Iboga, through its main compound ibogaine, has been linked in several real-world reports to the same kind of QT lengthening, sometimes seriously. Stacking them could add up, raising the risk of an irregular, dangerous heartbeat. Warning signs would be a racing or fluttering heart, dizziness, or fainting.

To be fair, this is rated on weak, anecdotal evidence, and it's more of a theoretical add-on effect than a proven one. Still, since ibogaine carries documented heart-rhythm dangers (and is illegal in the U.S.), please don't combine these without talking to your doctor or pharmacist first.

It carries serious safety risks, including dangerous heart rhythm changes and deaths, and ibogaine is illegal in the United States and many other countries. We do not recommend its use outside of tightly controlled research settings.

What Is Trimethoprim Used For?

Brand names: Proloprim, Trimpex

Trimethoprim tablets are used to treat initial episodes of uncomplicated urinary tract infections (UTIs). It works against specific bacteria — including E. coli, Proteus mirabilis, Klebsiella pneumoniae, Enterobacter species, and coagulase-negative Staphylococcus species (including S. saprophyticus).

It should only be used when there is a proven or strongly suspected bacterial infection. Your doctor may order a urine culture to confirm which bacteria are present and whether trimethoprim will work against them.
