Aliskiren falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Indian Long Pepper interacts with.

Theoretically, Indian long pepper might increase the effects and adverse effects of CYP3A4 substrates.
In vitro research shows that piperine, a constituent of Indian long pepper, inhibits CYP3A4.

Here's the gist with aliskiren (Tekturna). Your body partly relies on an enzyme called CYP3A4 to clear this blood pressure medicine. In lab studies, the piperine in Indian long pepper can slow that enzyme down. In theory, that could let a bit more aliskiren build up in your system, nudging its effect up a notch.

What might that look like in real life? Possibly more of aliskiren's usual effects, like lightheadedness, dizziness when you stand up, or your blood pressure dropping lower than planned. I'll be honest though, this is theoretical and based only on test-tube work, so I wouldn't lose sleep over an occasional sprinkle of the spice. If you take it as a daily supplement, just mention it to your pharmacist or doctor so they can keep an eye on your numbers.
