These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Indian Long Pepper interacts with whole categories of medication. Aspirin, Caffeine falls into 2 of those categories, so it can interact with Indian Long Pepper in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
1 of 2Why this applies to Aspirin, Caffeine
Aspirin, Caffeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Indian Long Pepper interacts with.
In vitro research shows that piperine, a constituent of Indian long pepper, inhibits CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo (think Anacin, which pairs aspirin with caffeine). Piperine, the punchy compound in Indian long pepper, can mildly slow down a liver enzyme called CYP3A4 in lab studies. Caffeine is partly handled by these pathways, so in theory long pepper could let caffeine linger a bit longer in your system. That might mean more of the usual caffeine effects, like feeling jittery, a racing heart, trouble sleeping, or an upset stomach.
I want to be straight with you though: this is theoretical, based on test-tube data, not real people. A pinch in cooking is very unlikely to cause problems. Concentrated supplement doses are more of a question mark, so just give your pharmacist or doctor a heads-up if you take it regularly.

ANTICOAGULANT/ANTIPLATELET DRUGS
2 of 2Aspirin, Caffeine also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.
In vitro research shows that Indian long pepper extract inhibits platelet aggregation.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing with Anacin: it pairs aspirin (a blood thinner that keeps platelets from clumping) with caffeine for a little extra pick-me-up. The piece worth watching is the aspirin. Lab studies suggest that Indian long pepper may also make platelets less sticky, so stacking it on top of aspirin could, in theory, nudge your bleeding risk a bit higher.
In real life that might show up as easier bruising, nosebleeds, bleeding gums, or a cut that takes longer to stop. I'd stress that this is theoretical and only based on test-tube research, not human studies, so it's far from certain. A sprinkle in cooking is unlikely to matter much. Still, if you take long pepper supplements regularly, give your pharmacist or doctor a heads-up first.

From our pharmacist’s overview on the Aspirin, Caffeine page
“Anacin combines aspirin with caffeine, so supplements can hit it from two directions, and a few of the 347 on this list deserve real respect. The biggest group, over 200 ingredients, can raise bleeding risk on top of aspirin's blood-thinning effect; beer and wine carry major ratings because alcohol plus an aspirin-type drug makes stoma…”
