Colchicine, Probenecid falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Indian Long Pepper interacts with.
In vitro research shows that piperine, a constituent of Indian long pepper, inhibits CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill pairs colchicine with probenecid to treat gout. The part I want to flag is the colchicine. Your body relies on an enzyme called CYP3A4 to help break colchicine down. In lab studies, the piperine in Indian long pepper can slow that same enzyme, so in theory colchicine could clear more slowly and build up in your blood.
Colchicine has a narrow safety window, meaning small increases can tip you into trouble. Higher levels can bring on nausea, vomiting, diarrhea, stomach cramps, and muscle weakness. To be clear, this concern comes from test-tube research, not human studies, and a sprinkle of this spice in food is unlikely to matter. Still, if you take it as a regular supplement, please run it by your pharmacist or doctor first.

P-GLYCOPROTEIN SUBSTRATES

This combo pill pairs colchicine with probenecid to treat gout. The part to watch here is the colchicine. Your body uses a pump called P-glycoprotein to help move colchicine out, and lab studies suggest piperine (the active compound in Indian long pepper) may slow that pump down. If that happens, colchicine could build up a bit more than usual.
Colchicine has a fairly narrow safety window, so higher levels can mean more nausea, diarrhea, stomach cramps, or muscle aches. Keep in mind this is theoretical, based on test-tube research, not on people, and a sprinkle in cooking is far less of a concern than concentrated capsules. Still, if you take this supplement regularly, give your pharmacist or doctor a heads-up first.
