Indian Long Pepper and Isoniazid, Rifampin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

RIFAMPIN (Rifadin)

Theoretically, Indian long pepper might increase blood levels of rifampin.
Piperine, a constituent of Indian long pepper, seems to increase absorption and serum levels of rifampin.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Rifamate is a combination tablet that pairs rifampin with isoniazid, two antibiotics used together to treat tuberculosis. The piece that matters here is the rifampin. The piperine in Indian long pepper may help your gut absorb more rifampin, which could nudge its blood levels a bit higher than usual.

If rifampin levels climb, you might notice more of its typical effects, like stomach upset, nausea, or in some cases signs of liver irritation (fatigue, loss of appetite, yellowing of the eyes or skin). I want to be honest though: this concern is theoretical, based on how the compounds behave rather than strong human studies. A sprinkle of pepper in food is unlikely to matter, but a concentrated supplement is worth mentioning. Please don't change anything on your own. Just let your doctor or pharmacist know if you take it regularly.
