PHENYTOIN (Dilantin)
A small pharmacokinetic study shows that piperine, a constituent of Indian long pepper, increases phenytoin serum levels and slows its elimination.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for mephenytoin (Mesantoin). It's an older seizure medicine that's chemically very close to phenytoin, and your body relies on liver enzymes to break it down and clear it out. The piperine in Indian long pepper can slow those enzymes down, which may let mephenytoin build up in your blood. We know phenytoin behaves this way from a small human study, and mephenytoin is similar enough that the same thing could plausibly happen.
If levels climb too high, you might notice things like dizziness, unsteady walking, slurred speech, drowsiness, or shaky eye movements. Seizure drugs like this have a narrow safe range, so even modest shifts matter. A pinch of pepper in cooking is unlikely to be the issue, but a concentrated supplement could be.
