Theoretically, Indian long pepper might increase blood levels of phenytoin.
A small pharmacokinetic study shows that piperine, a constituent of Indian long pepper, increases phenytoin serum levels and slows its elimination.

Here's the gist: the piperine in Indian long pepper can slow how fast your body clears phenytoin (Dilantin). A small human study found that piperine raised phenytoin levels in the blood and made it hang around longer. Phenytoin is a tricky one because it has a narrow window. The dose that controls seizures sits close to the dose that causes trouble, so even a modest bump can matter.

If levels climb too high, you might notice dizziness, drowsiness, slurred speech, unsteady walking, or jerky eye movements. I wouldn't change anything on your own. Just give your pharmacist or doctor a heads-up if you use this spice regularly as a supplement, since concentrated doses are more likely to matter than a sprinkle in cooking. They can check a level if needed.
