Theoretically, khella might have additive adverse effects on the liver when used with hepatotoxic drugs. Khella can increase liver transaminases. Some drugs that can adversely affect the liver include acetaminophen (Tylenol), amiodarone (Cordarone), carbamazepine (Tegretol), isoniazid (INH), methotrexate (Rheumatrex), methyldopa (Aldomet), and many others.

Here's the simple version: this isn't about khella changing how much mephenytoin is in your body. It's about both of them being potentially tough on your liver. Mephenytoin, an older seizure medicine, can sometimes irritate the liver on its own. Khella has been shown to nudge up liver enzymes (the blood markers that signal liver stress), so stacking the two together could theoretically add up and put extra strain on your liver.

To be fair, this is mostly a theoretical concern, not something proven to happen often. Still, watch for warning signs like yellowing of your skin or eyes, dark urine, belly pain, or unusual tiredness. Please loop in your pharmacist or doctor before combining these, since they may want to check your liver function.
