Levetiracetam falls under CNS DEPRESSANTS — the group Kratom interacts with.

Theoretically, concomitant use might increase the risk for adverse CNS depressant effects.
Kratom contains mitragynine, a mu-receptor agonist that may cause respiratory depression. Although results from animal research suggest that mitragynine causes less respiratory depression than the CNS depressant codeine, fatalities have been reported for patients who ingested a combination of kratom and O-desmethyltramadol, another mu-receptor agonist. Additionally, observational research suggests that adverse effects such as drowsiness and coma are more likely to occur in individuals taking kratom in combination with agents such as opioids and sedatives. Theoretically, ingesting kratom along with other CNS depressants can increase the risk of adverse effects, including drowsiness, coma, and/or severe or fatal respiratory depression.

Here's the simple version: both kratom and levetiracetam (Keppra) can have a calming, sedating effect on the brain. Keppra itself often causes drowsiness, fatigue, dizziness, and sometimes mood changes. Kratom acts on opioid receptors and can also make you sleepy and, at higher doses, slow your breathing. Put them together and those effects may stack, so you could feel more tired, foggy, or unsteady than usual.

I want to be honest about the evidence here: this concern is theoretical, based on how these substances behave rather than solid studies on this exact pair. There's no enzyme tug-of-war changing your Keppra levels, it's just overlapping sedation. Still, since deep drowsiness and breathing problems have been reported when kratom is mixed with other depressants, please talk with your doctor or pharmacist before combining them.

About Levetiracetam

What Is Levetiracetam Used For?

Brand names: Keppra, Elepsia XR, Spritam

Levetiracetam is used to treat several types of seizures. It treats partial-onset seizures (seizures that start in one area of the brain) and is used as add-on therapy for myoclonic seizures (sudden, brief muscle jerks) in people with juvenile myoclonic epilepsy aged 12 and older, and for primary generalized tonic-clonic seizures (grand mal seizures) in people with idiopathic generalized epilepsy aged 6 and older.

Age eligibility varies by product. Keppra oral and Roweepra tablets treat partial-onset seizures in patients as young as 1 month. Spritam and levetiracetam tablets for oral suspension treat partial-onset seizures in patients 4 years and older weighing more than 20 kg. Extended-release products — including Elepsia XR, Keppra XR, and Levetiracetam ER — are approved only for partial-onset seizures in patients 12 years and older. Injectable forms are used intravenously when a patient temporarily cannot take oral medication.
