Why this applies to Tofacitinib

Tofacitinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kratom interacts with.

Kratom might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that kratom extract inhibits CYP3A4 enzyme activity. A pharmacokinetic study in adults shows that taking kratom tea 2 grams modestly increases the plasma concentration time-curve (AUC) and maximum concentration (Cmax) of CYP3A4 probe midazolam by 40-50% and its CYP3A4-mediated metabolites 18-27%. However, a lack of change in the half-life of midazolam and a simulation of the kratom-midazolam interaction suggest that kratom inhibits CYP3A4 enzymes in the small intestine but not the liver. Additionally, there is one case report of a 27-year-old male who died from neuroleptic malignant-like symptoms after concomitant use of kratom and quetiapine, a CYP3A4 substrate. Although it did not appear that the patient had consumed large quantities of quetiapine, postmortem plasma levels were in the lethal range, indicating possible inhibition of CYP3A4 by kratom. In another case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It is hypothesized that CYP3A4 inhibition by kratom reduced metabolism of desvenlafaxine, trazodone, and ziprasidone and consequently increased systemic exposure to serotonin.

Here's the gist: your body partly uses an enzyme called CYP3A4 to clear tofacitinib (Xeljanz). Kratom appears to slow that enzyme down, mostly in the gut. So in theory, pairing the two could let a bit more tofacitinib build up in your system than expected.

If that happens, you might notice the medication's known downsides more, like a higher chance of infections, headaches, or changes in lab work such as blood counts and cholesterol. I want to be honest, though: this is a theoretical concern based mostly on lab studies, not on people taking these two together. Still, since kratom carries its own real risks and tofacitinib dampens your immune system, please check with your pharmacist or doctor before combining them.

Here's the gist for Tofacitinib (Xeljanz) and kratom. Your body clears tofacitinib mostly through a liver enzyme called CYP3A4, with some help from CYP2C19. In lab dish studies, kratom appears to weakly block CYP2C19. In theory, slowing that pathway could let a little more tofacitinib build up than usual, which might mean a slightly higher chance of side effects like headache, diarrhea, or a greater risk of infection.

That said, this is a theoretical concern based only on test-tube data, and it's rated minor. There's no real-world proof it causes problems. The bigger issue is kratom itself, which carries its own serious risks. Please loop in your pharmacist or doctor before combining the two.

What Is Tofacitinib Used For?

  • Tofacitinib is used alone or with other medications to treat rheumatoid arthritis (condition in which the body attacks its own joints causing pain, swelling, and loss of function) in adults who are unable to take or did not respond to one or more tumor necrosis factor (TNF) inhibitor medication(s). It is also used to treat psoriatic arthritis (a condition that causes joint pain and swelling and scales on the skin) in adults who are unable to take or did not respond to or tolerate one or more TNF inhibitor medication(s). Tofacitinib is used to treat ulcerative colitis (a condition which causes swelling and sores in the lining of the colon [large intestine] and rectum) in adults who are unable to take or who did not respond to one or more TNF inhibitor medication(s). It is also used to treat polyarticular juvenile idiopathic arthritis (PJIA; a type of childhood arthritis that affects five or more joints during the first six months of the condition, causing pain, swelling, and loss of function) in children 2 years and older who are unable to take or who did not respond to one or more TNF inhibitor medication(s). Tofacitinib is in a class of medications called Janus kinase (JAK) inhibitors. It works by decreasing the activity of the immune system.
