Captopril, Hydrochlorothiazide falls under ANTIHYPERTENSIVE DRUGS — the group L-arginine interacts with.

Theoretically, concomitant use of L-arginine and antihypertensive drugs may increase the risk of hypotension.
L-arginine increases nitric oxide, which causes vasodilation. Clinical evidence shows that L-arginine can reduce blood pressure in some individuals with hypertension. Furthermore, combining L-arginine with some antihypertensive drugs seems to have additive vasodilating and blood pressure-lowering effects.

Here's the simple version: your Capozide already lowers blood pressure two ways. Captopril relaxes blood vessels, and the hydrochlorothiazide ('water pill') sheds extra fluid and salt. L-arginine pushes your body to make more nitric oxide, which also relaxes and widens blood vessels. Stack them together and the effects can add up, so your pressure may drop a little more than expected.

If it dips too low, you might feel dizzy, lightheaded, tired, or get a headache, especially when standing up quickly. This concern is mostly theoretical, based on how these work rather than strong studies, so it's not a reason to panic. Still, if you'd like to try L-arginine, run it by your pharmacist or doctor first, and keep an eye on your blood pressure readings.

Captopril, Hydrochlorothiazide also falls under ACE INHIBITORS (ACEIs).

Theoretically, concomitant use of L-arginine and ACE inhibitors may increase the risk for hypotension and hyperkalemia.
Combining L-arginine with some antihypertensive drugs, especially ACE inhibitors, seems to have additive vasodilating and blood pressure-lowering effects. Furthermore, ACE inhibitors can increase potassium levels. Use of L-arginine has been associated with hyperkalemia in some patients. Theoretically, concomitant use of ACE inhibitors with L-arginine may increases the risk of hyperkalemia.

Here's the thing to keep in mind with your Capozide (that's captopril plus hydrochlorothiazide). Captopril already works by relaxing your blood vessels, and L-arginine pushes in the same direction by helping make nitric oxide, which widens vessels too. Stack them and the blood-pressure-lowering effect can add up, so you might feel lightheaded, dizzy, or woozy when you stand up, especially since the water pill can leave you a little dry on fluids.

There's a second piece: captopril tends to nudge potassium upward, and L-arginine has been linked to higher potassium in some people, so together they could raise it more (the hydrochlorothiazide partly offsets that by lowering potassium). I'll be honest, this is a theoretical concern based on how the drugs work, not strong proof. Still, please run it by your pharmacist or doctor before adding L-arginine.

What Is Captopril and Hydrochlorothiazide Used For?

Brand names: Capozide, Acezide

Captopril and hydrochlorothiazide tablets are used to treat high blood pressure (hypertension). The two ingredients work together to lower blood pressure more effectively than either one alone.

This combination may be started as initial therapy or substituted when a patient has already been adjusted on both ingredients separately. It is approved for patients with normal kidney function, and is used more cautiously in those with impaired kidney function.
