Belzutifan falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Licorice interacts with.

Theoretically, licorice might increase levels of drugs metabolized by CYP2C19.
In vitro, licorice extracts from the species G. glabra and G. uralensis inhibit CYP2C19 isoenzymes in vitro. Theoretically, these species of licorice might increase levels of drugs metabolized by CYP2C19; however, this interaction has not yet been reported in humans.

Here's the gist: your body relies on an enzyme called CYP2C19 to help clear belzutifan (Welireg) out of your system. In lab dishes, licorice extract appears to slow that same enzyme down. If that holds true in people, belzutifan could build up a bit more than expected, which might mean stronger side effects like anemia (low red blood cells), fatigue, dizziness, or low oxygen levels in the blood.

Now, the honest part: this is only seen in test-tube studies, never confirmed in actual patients, so it's more of a theoretical caution than a proven problem. Still, because belzutifan is a cancer medicine where the dose really matters, I'd loop in your oncologist or pharmacist before adding any licorice supplement. Occasional licorice candy is far less of a concern than concentrated daily capsules.

Licorice root is a traditional remedy used for sore throats, coughs, and digestive complaints, but solid human evidence is limited for most uses. Regular licorice contains glycyrrhizin, which can raise blood pressure and lower potassium when taken in large amounts or for a long time, so deglycyrrhizinated (DGL) products are often preferred. Talk to your pharmacist or doctor before using it, especially if you have high blood pressure, heart, or kidney problems.

What Is Belzutifan Used For?

Brand name: Welireg

Welireg treats several serious conditions. In adults with von Hippel-Lindau (VHL) disease, it targets associated kidney cancer (renal cell carcinoma), brain or spinal tumors (CNS hemangioblastomas), and pancreatic neuroendocrine tumors that don't need immediate surgery. It is also used in adults with clear cell renal cell carcinoma (ccRCC) — either after surgery to lower the risk of the cancer coming back (used with pembrolizumab or pembrolizumab and berahyaluronidase alfa-pmph), or as a single agent for advanced ccRCC that has already been treated with two other types of cancer drugs.

Welireg also treats pheochromocytoma or paraganglioma (PPGL) — rare tumors of the adrenal gland or nearby nerve tissue — in adults and in children 12 years and older when the tumor cannot be removed by surgery or has spread.
