Why Are There 3 Interactions?
Licorice interacts with whole categories of medication. Hydrocortisone falls into 3 of those categories, so it can interact with Licorice in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
1 of 3Why this applies to Hydrocortisone
Hydrocortisone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Licorice interacts with.
Pharmacokinetic research shows that the licorice constituent glycyrrhizin, taken in a dosage of 150 mg orally twice daily for 14 days, modestly decreases the area under the concentration-time curve of midazolam by about 20%. Midazolam is a substrate of CYP3A4, suggesting that glycyrrhizin modestly induces CYP3A4 activity. Animal research also shows that licorice extract from the species G. uralensis induces CYP3A4 activity. However, licorice extract from G. glabra species appear to inhibit CYP3A4-induced metabolism of testosterone in vitro. It is thought that the G. glabra inhibits CYP3A4 due to its constituent glabridin, which is a moderate CYP3A4 inhibitor in vitro and not present in other licorice species. Until more is known, licorice should be used cautiously in people taking CYP3A4 substrates.

Here's the deal with combining licorice and hydrocortisone. Your liver uses an enzyme called CYP3A4 to clear hydrocortisone out of your body. Licorice may nudge this enzyme in either direction depending on the type, so the research here is mixed and mostly theoretical. One study suggested licorice speeds the enzyme up a bit (which could slightly lower hydrocortisone's effect), while a lab study hinted the opposite.
On top of that, regular licorice (the kind with glycyrrhizin) acts a lot like a steroid itself, so it can stack with hydrocortisone to raise blood pressure, hold onto water, and drop your potassium. This part matters more than the enzyme question. Please check with your pharmacist or doctor before pairing them, especially if you have blood pressure, heart, or kidney concerns. DGL licorice avoids most of this.

In vitro research shows that licorice can increase P-glycoprotein activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with licorice and oral hydrocortisone: licorice may rev up a transporter pump in your gut called P-glycoprotein. When that pump works harder, it can shuttle some of the hydrocortisone back out before your body absorbs it. In theory, that could mean a little less of your dose actually getting in, which matters most if you take hydrocortisone to replace a hormone your body isn't making.
I want to be honest with you though: this is based only on lab and test-tube research, not real people, so the effect in everyday life is uncertain and probably small. The bigger concern with licorice is its glycyrrhizin, which can raise blood pressure and drop potassium, and steroids can do the same. Please check with your pharmacist or doctor before combining them, and DGL licorice avoids that issue.

CORTICOSTEROIDS

Case reports suggest that concomitant use of licorice and oral corticosteroids, such as hydrocortisone, can potentiate the duration of activity and increase blood levels of corticosteroids. Additionally, in one case report, a patient with neurogenic orthostatic hypertension stabilized on fludrocortisone 0.1 mg twice daily developed pseudohyperaldosteronism after recent consumption of large amounts of black licorice.

Here's the gist with licorice and hydrocortisone. Licorice slows down an enzyme (called 11-beta-HSD2) that normally helps your body clear cortisol-type steroids. Hydrocortisone is essentially the same family, so the worry is that it sticks around longer and builds up a bit more than it should. That can nudge up the usual steroid side effects, like fluid retention, puffiness, higher blood pressure, low potassium, and mood or sleep changes.
I'll be honest though, this is based mostly on a few case reports, not strong studies, and it usually involves regular use of real glycyrrhizin-containing licorice in larger amounts. A little licorice candy now and then is unlikely to be a problem. If you take hydrocortisone daily or use licorice supplements regularly, check with your pharmacist or doctor first.

Hydrocortisone comes in many products, and each one is approved for different uses. Which one you get depends on the condition being treated.
Cortef tablets are used for adrenal insufficiency, congenital adrenal hyperplasia, rheumatic disorders (such as rheumatoid arthritis and gouty arthritis) and collagen diseases such as lupus. Hydrocortisone sodium succinate and Solu-Cortef injections are used when oral therapy is not feasible, for severe allergic states, skin diseases, and adrenal insufficiency. Alkindi (oral granules) is used as replacement therapy in children with adrenocortical insufficiency. Cortenema, Hydrocortisone Rectal Suspension and Cortifoam are used for ulcerative colitis or ulcerative proctitis. Topical hydrocortisone butyrate and valerate products, including Evesse lotion, are used for itchy, inflamed skin conditions and atopic dermatitis.
