CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Beclometasone dipropionate

Beclometasone dipropionate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Licorice interacts with.

Theoretically, licorice might increase or decrease levels of drugs metabolized by CYP3A4.
Pharmacokinetic research shows that the licorice constituent glycyrrhizin, taken in a dosage of 150 mg orally twice daily for 14 days, modestly decreases the area under the concentration-time curve of midazolam by about 20%. Midazolam is a substrate of CYP3A4, suggesting that glycyrrhizin modestly induces CYP3A4 activity. Animal research also shows that licorice extract from the species G. uralensis induces CYP3A4 activity. However, licorice extract from G. glabra species appear to inhibit CYP3A4-induced metabolism of testosterone in vitro. It is thought that the G. glabra inhibits CYP3A4 due to its constituent glabridin, which is a moderate CYP3A4 inhibitor in vitro and not present in other licorice species. Until more is known, licorice should be used cautiously in people taking CYP3A4 substrates.

Here's the thing about your Becotide inhaler and licorice: most of the medicine works right in your lungs, and only small amounts reach the bloodstream. The liver enzyme called CYP3A4 helps clear what does get absorbed. Licorice can nudge that enzyme either way depending on the type, so honestly the effect on an inhaled steroid is likely to be quite small.

This is mostly theoretical and based on weak evidence, so I wouldn't lose sleep over it. My bigger concern is licorice itself. Real glycyrrhizin licorice can raise blood pressure and lower potassium, which isn't great alongside steroid medicines. If you use licorice regularly, choose a DGL product and check in with your pharmacist or doctor first.

Here's the gist with licorice and your beclometasone (Becotide) inhaler. Lab studies suggest licorice may rev up a pump called P-glycoprotein, which can push certain medicines back out of cells and lower how much your body absorbs. In theory, that could mean a little less beclometasone gets where it needs to go, which might soften how well it controls airway inflammation.

I'll be honest, though, this is based only on test-tube and animal research, so it's far from proven. Beclometasone is also inhaled and works mostly right in your lungs, so a gut-absorption effect probably matters less here. Don't change your inhaler routine on your own. If you notice more wheezing or breathlessness, check in with your pharmacist or doctor.

CORTICOSTEROIDS

Beclometasone dipropionate also falls under CORTICOSTEROIDS.

Theoretically, concomitant use of licorice and corticosteroids might increase the side effects of corticosteroids.
Case reports suggest that concomitant use of licorice and oral corticosteroids, such as hydrocortisone, can potentiate the duration of activity and increase blood levels of corticosteroids. Additionally, in one case report, a patient with neurogenic orthostatic hypertension stabilized on fludrocortisone 0.1 mg twice daily developed pseudohyperaldosteronism after recent consumption of large amounts of black licorice.

Here's the thing to know about beclometasone (Becotide): you're usually inhaling it for asthma or breathing problems, so very little reaches your bloodstream compared with steroid pills. The concern on file comes mostly from oral steroids, where licorice can slow how the body clears the steroid and let levels build up. With an inhaler, that effect is much smaller.

The bigger worry is that licorice (the glycyrrhizin kind, not DGL) can on its own cause the body to hold onto sodium and water and lose potassium, which raises blood pressure and can puff you up. Steroids can nudge things the same direction, so the two together might add up. This is theoretical and based on case reports, so don't panic, but do check with your pharmacist or doctor before regular licorice use.
