CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Prednisolone

Prednisolone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Licorice interacts with.

Theoretically, licorice might increase or decrease levels of drugs metabolized by CYP3A4.
Pharmacokinetic research shows that the licorice constituent glycyrrhizin, taken in a dosage of 150 mg orally twice daily for 14 days, modestly decreases the area under the concentration-time curve of midazolam by about 20%. Midazolam is a substrate of CYP3A4, suggesting that glycyrrhizin modestly induces CYP3A4 activity. Animal research also shows that licorice extract from the species G. uralensis induces CYP3A4 activity. However, licorice extract from G. glabra species appear to inhibit CYP3A4-induced metabolism of testosterone in vitro. It is thought that the G. glabra inhibits CYP3A4 due to its constituent glabridin, which is a moderate CYP3A4 inhibitor in vitro and not present in other licorice species. Until more is known, licorice should be used cautiously in people taking CYP3A4 substrates.

Here's the thing with licorice and prednisolone: your body uses an enzyme called CYP3A4 to help clear this steroid. Licorice can pull in two directions depending on the species and the product. Some forms appear to speed that enzyme up (which could lower your prednisolone levels a bit and weaken its effect), while others may slow it down (letting the steroid build up, which can mean more puffiness, trouble sleeping, higher blood sugar, or mood changes). Prednisolone is the already-active drug, not a prodrug, so it simply rises or falls with these shifts.

Honestly, the evidence here is fairly weak and the effect looks modest. The bigger concern is separate: real licorice with glycyrrhizin can raise blood pressure and drop potassium, and steroids do the same, so together they add up. A DGL product avoids that. Please check with your pharmacist or doctor before pairing them.

Here's the gist for prednisolone. Lab studies (in test tubes and animals) suggest licorice may rev up a transporter called P-glycoprotein, which acts like a pump that pushes certain drugs back out of your gut. If that pump works harder, your body might absorb a little less prednisolone, which could make it slightly less effective. Prednisolone is the active drug, not a prodrug, so this would mean weaker effect, not stronger.

Honestly, this is theoretical and only based on lab data, so I wouldn't lose sleep over it. A bit of licorice candy or tea is unlikely to matter much. Concentrated, long-term licorice supplements are the bigger concern, partly because licorice can also raise blood pressure and lower potassium, which steroids already do. Check with your pharmacist or doctor before combining them.

Theoretically, concomitant use of licorice and corticosteroids might increase the side effects of corticosteroids.
Case reports suggest that concomitant use of licorice and oral corticosteroids, such as hydrocortisone, can potentiate the duration of activity and increase blood levels of corticosteroids. Additionally, in one case report, a patient with neurogenic orthostatic hypertension stabilized on fludrocortisone 0.1 mg twice daily developed pseudohyperaldosteronism after recent consumption of large amounts of black licorice.

Here's the deal with mixing licorice and prednisolone. Licorice (the kind with glycyrrhizin, not DGL) can slow down how quickly your body clears prednisolone, so the steroid may hang around longer and feel a bit stronger. That can mean more of the usual prednisolone side effects, like fluid buildup, puffiness, higher blood sugar, mood or sleep changes, and a bump in blood pressure.

On top of that, licorice itself causes your body to hold onto sodium and lose potassium, which pulls in the same direction as the steroid. Honestly though, this is based on a few case reports and theory, mostly with heavy or long-term licorice use, so a little flavoring is unlikely to matter. Concentrated supplements or lots of black licorice are the real concern. Please check with your pharmacist or doctor before combining them.

Prednisolone is prescribed for many different conditions where inflammation or an overactive immune system is causing harm. These range from severe allergic reactions and asthma to certain skin diseases, blood disorders, eye problems, and some cancers.

It is also used to support the body when the adrenal glands aren't making enough of their own steroids, and to manage flares of conditions like ulcerative colitis, multiple sclerosis, and nephrotic syndrome (a kidney condition causing protein loss in the urine).
