Here's the simple version: your body uses a liver enzyme called CYP2C19 to help process enasidenib (Idhifa). In test-tube studies, certain licorice extracts can slow that enzyme down. If that happened in real life, enasidenib might clear out a little more slowly and build up, which could mean more of its known side effects, things like nausea, low appetite, fatigue, or changes in your liver and bilirubin levels.

I want to be honest about how soft this evidence is. It's only been seen in lab studies, never reported in actual patients, so it's more of a theoretical concern than a proven problem. Still, since enasidenib is a serious cancer medicine, please don't add licorice supplements without checking with your oncology team or pharmacist first. They know your full picture.

There is conflicting evidence about the effect of licorice on CYP2C9 enzyme activity. In vitro research shows that extracts from the licorice species G. glabra and G. uralensis moderately inhibit CYP2C9 isoenzymes. However, evidence from an animal model shows that licorice extract from the species G. uralensis can induce hepatic CYP2C9 activity. Until more is known, licorice should be used cautiously in people taking CYP2C9 substrates.

Here's the honest picture with licorice and your leukemia medicine, enasidenib (Idhifa). Enasidenib is partly handled by a liver enzyme called CYP2C9, and licorice might nudge that enzyme one way or the other. The catch is that the evidence is shaky and conflicting. Some lab studies suggest licorice slows this enzyme (which could let more drug build up), while an animal study suggests it speeds it up (which could lower drug levels). We honestly don't know which wins in real people.

If levels crept up, you might see more nausea, diarrhea, or low appetite. If they dropped, the medicine could work less well. Because this is theoretical and the stakes are high with cancer treatment, please don't add licorice supplements without checking with your oncology team or pharmacist first. Occasional licorice candy is a different, much smaller concern.

Theoretically, licorice might increase or decrease levels of drugs metabolized by CYP3A4.
Pharmacokinetic research shows that the licorice constituent glycyrrhizin, taken in a dosage of 150 mg orally twice daily for 14 days, modestly decreases the area under the concentration-time curve of midazolam by about 20%. Midazolam is a substrate of CYP3A4, suggesting that glycyrrhizin modestly induces CYP3A4 activity. Animal research also shows that licorice extract from the species G. uralensis induces CYP3A4 activity. However, licorice extract from G. glabra species appear to inhibit CYP3A4-induced metabolism of testosterone in vitro. It is thought that the G. glabra inhibits CYP3A4 due to its constituent glabridin, which is a moderate CYP3A4 inhibitor in vitro and not present in other licorice species. Until more is known, licorice should be used cautiously in people taking CYP3A4 substrates.

Here's the situation with Idhifa (enasidenib) and licorice. Your leukemia medicine is partly cleared by a liver enzyme called CYP3A4, and licorice can nudge that enzyme in either direction depending on the species and product. Most of the human data points to licorice mildly speeding up CYP3A4, which could lower enasidenib levels a bit and, in theory, make it work less well against the cancer. Other lab data hints it could do the opposite and raise levels.

Honestly, this is mostly theoretical and the effect seen in studies was modest. But since enasidenib is treating a serious illness, I wouldn't add a licorice supplement without first checking with your oncology team or pharmacist. Occasional licorice candy is a different story from concentrated daily supplements.

Here's the gist for enasidenib (Idhifa), your leukemia pill. Licorice may gently speed up a liver enzyme called CYP1A2, which is one pathway that helps process this drug. In theory, that could clear enasidenib a little faster and slightly soften how well it works. Enasidenib is the active drug, not a prodrug, so this points toward less effect rather than more.

I want to be straight with you, though: this concern comes only from lab test-tube studies, not from people, and it's rated minor. With a cancer medicine like this, I wouldn't gamble on any unknown. Licorice root is a traditional remedy used for sore throats, coughs, and digestive complaints, but solid human evidence is limited for most uses. Regular licorice contains glycyrrhizin, which can raise blood pressure and lower potassium when taken in large amounts or for a long time, so deglycyrrhizinated (DGL) products are often preferred. Idhifa (enasidenib) is used to treat adult patients with acute myeloid leukemia (AML) — a fast-moving cancer of the blood and bone marrow — whose disease has come back after treatment (relapsed) or has stopped responding to treatment (refractory). It is specifically for patients whose leukemia has a mutation in the IDH2 gene, which must be confirmed by an FDA-approved test before starting therapy.
