Why this applies to Aliskiren

Aliskiren falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Methoxylated Flavones interacts with.

In vitro evidence suggests that methoxylated flavones might inhibit cytochrome P450 3A4 (CYP3A4). This effect seems to be structure-dependent, and does not occur with all methoxylated flavones. So far this interaction has not been reported in humans. Theoretically, concurrent use of certain methoxylated flavones with drugs metabolized by CYP3A4 might result in increased drug levels and an increased risk for adverse effects.
Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), Pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline (Slo-bid, Theo-Dur, others), zileuton (Zyflo), Zolmitriptan (Zomig), and others.

Here's the gist for aliskiren (Tekturna). Some methoxylated flavones from citrus peel may block an enzyme called CYP3A4, which is one of the pathways your body uses to clear this blood pressure medicine. If that pathway slows down, aliskiren could build up a bit more than expected. In real life, that might mean a stronger drop in blood pressure, with effects like dizziness, lightheadedness, or feeling faint, especially when you stand up.

The good news: this is theoretical, based only on lab studies, and it hasn't been reported in people. Not every methoxylated flavone causes it either. Everyday citrus in food is very unlikely to matter, but concentrated supplements are less predictable. If you want to use one, check with your pharmacist or doctor first.

Aliskiren tablets — available as the brand Tekturna and as generic aliskiren — are used to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering blood pressure reduces the risk of strokes and heart attacks.

Controlling blood pressure is most effective as part of a broader plan that may include healthy eating, exercise, reducing sodium, and managing other risk factors like cholesterol and blood sugar. Many people need more than one blood pressure medicine to reach their goal.
