STIMULANT DRUGS
Why this applies to Acetaminophen, Butalbital, Caffeine
Acetaminophen, Butalbital, Caffeine falls under STIMULANT DRUGS — the group Methylsynephrine interacts with.
Some stimulant drugs include amphetamine, caffeine, diethylpropion (Tenuate), methylphenidate, phentermine (Ionamin), pseudoephedrine (Sudafed, others), and many others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the piece that matters for your headache medicine: the caffeine inside this combination is itself a stimulant. Methylsynephrine is also a stimulant, acting like adrenaline to push your heart rate and blood pressure up. Stacking two stimulants together can add up, so the concern is mostly about your heart and blood pressure, not about how the acetaminophen or butalbital get broken down.
In real life, that could mean a racing or pounding heart, jitteriness, anxiety, trouble sleeping, or a spike in blood pressure. This is a theoretical caution based on how these ingredients work, rated moderate, so I don't want to alarm you. Still, methylsynephrine isn't an approved supplement and is best avoided. Please run this by your pharmacist or doctor before combining them.

“Alcohol is the interaction I would flag first for anyone taking acetaminophen, butalbital, and caffeine (Esgic, Fiogesic, and others). Drinking raises the risk of liver injury from the acetaminophen component, and 38 of the 252 supplement ingredients on file carry that same liver concern.”

About Methylsynephrine
What is Methylsynephrine?
- Weight loss and fat burning
- Boosting energy
- Athletic and workout performance
- Appetite control
Methylsynephrine (also called oxilofrine) is a synthetic stimulant sometimes added to weight-loss and pre-workout products, but it is not a legal or approved dietary ingredient in the United States. It has been linked to serious heart and blood pressure problems, and there is no good evidence it is safe or effective for these uses, so it is best avoided.
About Acetaminophen, Butalbital, and Caffeine
What Is Acetaminophen, Butalbital, and Caffeine Used For?
Brand names: Tecnal, Esgic Plus, Fiogesic, Triad, Repan, Esgic, Fioricet
This three-ingredient combination — butalbital, acetaminophen, and caffeine — is used to relieve the symptoms of tension headache (also called muscle contraction headache). All supported products, including tablets, capsules, and oral solution, share this same approved use.
It is worth knowing that evidence for using this combination to treat repeated, frequent tension headaches over time is limited. Because butalbital can be habit-forming, extended or repeated use is not recommended.
