Phellodendron contains berberine. Animal research suggests that berberine may have sedative effects. Theoretically, phellodendron might also have CNS depressants effects.

This combination product contains codeine, an opioid that already causes drowsiness, slowed breathing, and a foggy head. The concern here is simple: Phellodendron contains berberine, which in animal studies seemed to have a mild calming or sedative effect. Stacking that on top of codeine could, in theory, make you sleepier or more sluggish than you expect.
I want to be honest about the strength of this. It's theoretical, based on how these compounds behave in the lab and in animals, not from real cases in people. The caffeine in this product may even offset some sedation. Still, if you ever feel unusually drowsy or notice your breathing seems slow, that's worth attention. Check with your pharmacist or doctor before pairing the two, especially if you take other sedating medicines.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Phellodendron contains berberine. In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, phellodendron might also inhibit CYP2D6.

Here's the wrinkle with this combo product: the codeine part is what matters most. Codeine is a prodrug, meaning your body has to switch it on using an enzyme called CYP2D6 to turn it into morphine, which is what actually relieves pain. Phellodendron contains berberine, which may slow that same enzyme down. So in theory, less codeine would get converted, and you could get weaker pain relief than expected.
I want to be straight with you though: this is based on lab work and pharmacology reasoning, not solid human studies, so it may not amount to much in real life. The caffeine, acetaminophen, and salicylamide pieces aren't the concern here. If you notice your pain isn't being controlled, check in with your pharmacist or doctor before changing anything.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

This combo product has a few ingredients, but the one to watch with Phellodendron is the codeine. Codeine is what we call a prodrug, meaning your body has to convert it into an active painkiller (morphine) before it does much. The handoff mostly runs through a liver enzyme called CYP2D6, with CYP3A4 playing a side role. Phellodendron (through its berberine content) may gently slow CYP3A4.
In plain terms, this is a theoretical concern based on lab work, not strong human data, so I wouldn't lose sleep over it. At most, nudging this pathway could shift how much codeine becomes active, slightly changing pain relief or side effects like drowsiness, constipation, or nausea. Before pairing them, just run it by your pharmacist or doctor.
