Inotersen falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Phellodendron interacts with.

Theoretically, phellodendron might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Phellodendron contains berberine. In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, phellodendron might also inhibit platelet aggregation.

Here's the thing to keep in mind with Inotersen (Tegsedi). This medicine treats a rare nerve condition (hereditary transthyretin amyloidosis), and on its own it can lower your platelet count. Platelets are the blood cells that help you clot, so low platelets already raise your bleeding risk, which is why your doctor checks your platelet levels regularly.

Phellodendron contains berberine, and in lab and animal studies berberine can make platelets less sticky. So in theory, stacking it on top of Inotersen could add to the bleeding tendency, meaning easier bruising, nosebleeds, bleeding gums, or harder-to-stop bleeding. I'll be honest, this is a theoretical concern based on lab work, not proven in people. Still, given that Inotersen already affects platelets, it's worth running this past your doctor or pharmacist before combining them.
