Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP2C9.
Phellodendron contains berberine. Preliminary clinical research shows that berberine can inhibit CYP2C9. Theoretically, phellodendron might also inhibit CYP2C9.

Here's the simple version. Aleve Cold and Sinus pairs naproxen (a pain and inflammation reliever) with pseudoephedrine (a decongestant). The naproxen part is cleared from your body by a liver enzyme called CYP2C9. Phellodendron contains berberine, which lab and early human research suggests can slow that enzyme down. So in theory, naproxen could clear a little slower and build up, nudging up the chance of side effects like stomach upset, heartburn, or, with longer use, stomach irritation or bleeding.

Keep in mind this is theoretical, based on how the drug is processed rather than real-world reports, so it may not amount to much. Still, since both also can affect the stomach, it's worth a quick chat with your pharmacist before combining them.

Theoretically, phellodendron might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Phellodendron contains berberine. In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, phellodendron might also inhibit platelet aggregation.

Here's the thing to know about Aleve Cold and Sinus: the naproxen part is already an NSAID, and NSAIDs make platelets a little less sticky on their own. That's why naproxen can cause stomach upset, easy bruising, or, less often, stomach bleeding. Phellodendron contains berberine, which in lab and animal studies also seems to slow platelets down. Stack the two together and, in theory, you could nudge your bleeding risk a bit higher.

I'll be honest, this concern comes only from test-tube and animal data, not real patients, so it's more of a caution than a proven problem. Still, if you notice unusual bruising, nosebleeds, or dark stools, or if you use blood thinners, check in with your pharmacist or doctor before combining them.
