Ibrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the gist for Imbruvica (ibrutinib). Your body relies heavily on a liver enzyme called CYP3A4 to clear this medicine out. Phellodendron contains berberine, which lab studies and some early human research suggest can slow that enzyme down. If the enzyme works slower, ibrutinib can build up higher than intended in your blood.

Why that matters: more ibrutinib can mean more of its known side effects, like easy bruising or bleeding, diarrhea, irregular heartbeat, or higher blood pressure. Ibrutinib is not a prodrug, so this points toward a stronger effect, not a weaker one. That said, this concern is theoretical and based mostly on lab work. Please don't stop or start anything on your own. Check with your oncology team or pharmacist first.

Phellodendron contains berberine. In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, phellodendron might also inhibit platelet aggregation.

Here's the thing worth knowing if you're on ibrutinib (Imbruvica): this cancer medicine already comes with a built-in bleeding risk on its own. It works partly by affecting platelets, so people taking it can bruise more easily, get nosebleeds, or bleed longer from small cuts.

Phellodendron contains berberine, and in lab and animal studies that compound appears to slow platelets down too. So the concern is simply that you'd be stacking two things that nudge bleeding in the same direction. This is theoretical, based on test-tube and animal data, not human studies, so it's not a reason to panic. Still, because ibrutinib's own bleeding risk is real, I'd run any phellodendron product by your oncologist or pharmacist before starting it.

What Is Ibrutinib Used For?

Brand name: Imbruvica

Ibrutinib (sold as Imbruvica) treats certain blood cancers in adults. These are chronic lymphocytic leukemia (CLL)/small lymphocytic lymphoma (SLL), including CLL/SLL with 17p deletion, and Waldenström’s macroglobulinemia (WM).

Imbruvica is also used for chronic graft versus host disease (cGVHD) in adults and children age 1 year and older. It is for people whose cGVHD did not improve after one or more other systemic treatments.
