Macitentan falls under ANTIHYPERTENSIVE DRUGS — the group Phellodendron interacts with.
Phellodendron contains berberine. Animal research suggests that berberine can have hypotensive effects. Also, a clinical study suggests that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. Theoretically, phellodendron might also reduce blood pressure.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: Phellodendron contains berberine, which may gently lower blood pressure on its own. Macitentan (Opsumit) already relaxes and opens up blood vessels to treat the high pressure in your lung arteries. Stack the two and the blood-pressure-lowering effect could add up a bit more than expected.
In real life, that might show up as feeling lightheaded, dizzy, or faint, especially when you stand up quickly. I want to be honest though: this concern is theoretical, based mostly on how berberine behaves in the lab and in animals, not on strong human studies with macitentan. It's not a reason to panic. Still, since PAH is serious, please loop in your pharmacist or doctor before adding phellodendron so they can keep an eye on your blood pressure.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea: your body clears macitentan using a liver enzyme called CYP3A4. Phellodendron contains berberine, which lab studies and some early human research suggest can mildly slow that same enzyme down. If the enzyme works slower, macitentan could linger in your system at higher levels than usual. Macitentan is an active drug (not a prodrug), so higher levels mean a stronger effect and a greater chance of side effects like low blood pressure, headache, swelling in your legs, stuffy nose, or feeling unusually tired.
I want to be honest, though: this is a theoretical concern based mostly on lab work, not proven in real patients. Still, before adding phellodendron, check with your pharmacist or doctor so they can weigh it for you.

What Is Macitentan Used For?
Brand name: Opsumit
Macitentan tablets — sold under the brand name Opsumit — are used to treat pulmonary arterial hypertension (PAH, WHO Group I), a type of high blood pressure in the arteries of the lungs. The goal is to slow the disease from getting worse and to lower the chance of being hospitalized because of PAH.
In the key clinical study, macitentan was used in adults with mainly moderate-to-severe PAH symptoms. Patients had PAH caused by unknown or inherited causes, connective tissue disorders, or congenital heart disease with repaired shunts.
