Macitentan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Dhea interacts with.

Theoretically, DHEA might increase the levels of drugs metabolized by CYP3A4.
Some preliminary evidence shows that DHEA may inhibit CYP3A4; however, the clinical significance of this potential interaction is not known.

Here's the gist for you. Your body clears Opsumit (macitentan) mainly through a liver enzyme called CYP3A4. There's some early lab-based hint that DHEA might slow that enzyme down. If it did, macitentan could build up a bit more than usual, which in theory might make side effects more noticeable, things like headache, low blood pressure, fluid buildup and swelling, a stuffy nose, or feeling more tired or anemic.

I want to be honest with you though: this is theoretical and based only on weak, anecdotal evidence, so we really don't know if it matters in real life. Macitentan isn't a prodrug, so the direction here points toward a stronger effect, not a weaker one. Before pairing these, please run it by your pharmacist or doctor.

What Is Macitentan Used For?

Brand name: Opsumit

Macitentan tablets — sold under the brand name Opsumit — are used to treat pulmonary arterial hypertension (PAH, WHO Group I), a type of high blood pressure in the arteries of the lungs. The goal is to slow the disease from getting worse and to lower the chance of being hospitalized because of PAH.

In the key clinical study, macitentan was used in adults with mainly moderate-to-severe PAH symptoms. Patients had PAH caused by unknown or inherited causes, connective tissue disorders, or congenital heart disease with repaired shunts.
