Tibolone falls under ESTROGENS — the group Dhea interacts with.

Theoretically, DHEA might increase the effects and adverse effects of estrogen therapy.
DHEA is a precursor to estrogen and androgen and is metabolized into those substances. In clinical research, DHEA supplements increase the levels of these hormones. Also, in clinical research, estrogen-progestin oral contraceptives and conjugated estrogens reduce blood levels of DHEA and DHEA-S. The clinical significance of these findings is unclear.

Here's the simple version. Tibolone (Livial) is a hormone medicine that acts in your body a lot like estrogen, progesterone, and a touch of testosterone all rolled into one. DHEA is a hormone building block your body turns into estrogen and androgens. So when you take both, you may be adding a little extra hormone activity on top of what tibolone already gives you.

In theory, this could nudge up tibolone-type effects and side effects, things like breast tenderness, spotting or vaginal bleeding, bloating, or mood changes. But honestly, this concern comes only from lab and limited research, not solid human studies, so it's rated minor and may not matter much. Still, because both are hormones, please run it by your pharmacist or doctor before combining them.
