Why this applies to Macitentan
Macitentan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bacopa interacts with.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body uses an enzyme called CYP3A4 to clear macitentan (Opsumit) out of your system. In test-tube studies, bacopa might slow that enzyme down a bit. If it does, macitentan could build up a little more than expected, which in theory could mean stronger effects like low blood pressure, headache, a stuffy nose, swelling in your legs, or fatigue.
That said, the science here is weak and conflicting. This is based only on lab studies, not on real people, and some of that lab work suggests no meaningful effect at all. So I wouldn't panic. Since macitentan is a serious medication for a serious condition, just give your pharmacist or doctor a heads-up before adding bacopa so they can keep an eye on you.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. What Is Macitentan Used For?
Brand name: Opsumit
Macitentan tablets — sold under the brand name Opsumit — are used to treat pulmonary arterial hypertension (PAH, WHO Group I), a type of high blood pressure in the arteries of the lungs. The goal is to slow the disease from getting worse and to lower the chance of being hospitalized because of PAH.
In the key clinical study, macitentan was used in adults with mainly moderate-to-severe PAH symptoms. Patients had PAH caused by unknown or inherited causes, connective tissue disorders, or congenital heart disease with repaired shunts.
