Ivacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP3A4 substrates.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down ivacaftor (Kalydeco). In lab dish studies, bacopa might slow that enzyme down a bit. If it does, ivacaftor could clear out more slowly and build up a little, which in theory could nudge up side effects like headache, dizziness, belly pain, or changes in liver-related lab numbers.

The honest part: this is only theoretical, based on test-tube data, and even those studies disagree with each other. We really don't know if it matters in real people. Ivacaftor isn't a prodrug, so blocking CYP3A4 would mean more drug, not less. Since cystic fibrosis treatment is too important to gamble with, please run any bacopa supplement past your CF team or pharmacist before starting it.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. What Is Ivacaftor Used For?

Brand name: Kalydeco

Kalydeco (ivacaftor) is used to treat cystic fibrosis (CF) in patients aged 1 month and older who have at least one mutation in the CFTR gene that responds to ivacaftor. A specific genetic test is used to confirm whether a patient's mutation qualifies.

There is a long list of CFTR mutations for which Kalydeco is approved — including G551D, R117H, F508C, S549N, and many others. If your or your child's mutation is unknown, an FDA-cleared CF mutation test should be done first to confirm eligibility.
