Lacosamide falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP2C9 substrates.
Research on the effect of bacopa extracts on CYP2C9 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and non-competitively inhibit CYP2C9, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the gist with bacopa and your Vimpat (lacosamide). Lacosamide is partly handled by a liver enzyme called CYP2C9. In test-tube studies, bacopa has shown a possible ability to slow that enzyme down. If that happened in your body, lacosamide could clear a little more slowly and build up, which might mean more of its usual side effects like dizziness, drowsiness, blurry or double vision, headache, or feeling unsteady on your feet.

I want to be honest though: this is theoretical. The lab evidence is mixed, and some of it suggests no meaningful effect at all. So this is more of a 'keep an eye out' situation than a red alert. Lacosamide isn't a prodrug, so it doesn't flip the other way. If you'd like to add bacopa, just check with your pharmacist or neurologist first.

Here's the gist for combining bacopa with your lacosamide (Vimpat). Lab studies hint that bacopa might mildly slow down CYP3A4, one of the liver enzymes that helps clear lacosamide. I want to be honest, though: this is only a theory from test tube research, and even that evidence is mixed. Lacosamide is also handled by more than one enzyme, so any change is likely small. Bacopa doesn't turn lacosamide into something stronger by activating it, so we're just talking about possible buildup. Still, since seizure control matters, mention bacopa to your pharmacist or doctor before starting it.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. It is generally well tolerated but can cause stomach upset, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.

What Is Lacosamide Used For?

Brand names: Vimpat, Motpoly XR

Lacosamide is used to treat partial-onset seizures (seizures that start in one part of the brain) in adults and children. The age at which it is approved depends on the specific product: Vimpat tablets, oral solution, and injection are approved for partial-onset seizures in patients as young as 1 month old; most other formulations are approved from age 4 and up; and Motpoly XR is approved for patients weighing at least 50 kg.

Lacosamide is also used as adjunctive (add-on) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain) in adults and children 4 years of age and older, including Motpoly XR for patients weighing at least 50 kg.
