Piroxicam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP2C9.
Phellodendron contains berberine. Preliminary clinical research shows that berberine can inhibit CYP2C9. Theoretically, phellodendron might also inhibit CYP2C9.

Here's the gist for Feldene (piroxicam). Your liver relies on an enzyme called CYP2C9 to clear this medicine out of your body. Phellodendron contains berberine, which early research suggests can slow that same enzyme down. If the enzyme works slower, piroxicam can hang around longer and build up, which may turn up its typical side effects.

For an NSAID like piroxicam, that could mean more stomach upset, heartburn, a higher risk of stomach irritation or bleeding, fluid retention, or kidney strain. I want to be honest though: this is a theoretical concern based on how the drug is processed, not proven harm in real patients. Still, please run it by your pharmacist or doctor before combining them.

Theoretically, phellodendron might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Phellodendron contains berberine. In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, phellodendron might also inhibit platelet aggregation.

Here's the thing to keep in mind with Piroxicam (Feldene). Like other NSAIDs, it already nudges up your bleeding risk a bit because it interferes with how platelets clump together. Phellodendron contains berberine, and lab and animal studies hint that berberine may also make platelets less sticky. Stack the two and, in theory, you could see more easy bruising, nosebleeds, stomach upset, or rarely a stomach bleed showing up as dark, tarry stools.

I want to be honest though: this concern comes only from test-tube and animal research, not from solid human studies, so it's more of a caution than a proven problem. Still, before you pair them, check with your pharmacist or doctor, especially if you also take a blood thinner or have a history of ulcers.
