Metformin Hydrochloride falls under ANTIDIABETES DRUGS — the group Phellodendron interacts with.

Theoretically, phellodendron may increase the risk of hypoglycemia when taken with antidiabetes drugs.
Phellodendron contains berberine. Clinical research shows that berberine may lower blood glucose levels. Theoretically, phellodendron might also lower blood glucose levels.

Here's the simple version: Phellodendron contains berberine, and berberine has been shown in studies to nudge blood sugar downward on its own. Your metformin is already working to lower your blood sugar, so stacking the two together could theoretically push it lower than you want. That's the concern here.

If blood sugar dips too low, you might feel shaky, sweaty, dizzy, lightheaded, or unusually hungry or irritable. Now, keep in mind this is a theoretical interaction based on how berberine behaves, not strong proof that it causes problems with metformin specifically. Still, it's worth a heads-up. Please don't change your metformin on your own. Just check with your pharmacist or doctor before adding phellodendron, and watch how you feel if you do.

Theoretically, phellodendron might increase the therapeutic and adverse effects of metformin.
Phellodendron contains berberine. In vitro and animal studies show that berberine can increase the systemic exposure and half-life of metformin, potentially increasing metformin's effects and side effects. This interaction seems to be most apparent when berberine is administered 2 hours prior to metformin. Taking berberine and metformin at the same time does not appear to increase systemic exposure to metformin. It is unclear if phellodendron might have this same effect.

Here's the gist for your metformin. Phellodendron bark contains berberine, and lab and animal studies hint that berberine may slow how fast your body clears metformin. In theory, that could let a little more metformin build up than usual, nudging both its blood-sugar-lowering effect and its side effects a bit higher. With metformin that can mean more stomach upset, nausea, diarrhea, or a bigger drop in blood sugar, especially if you also take other diabetes meds.

I want to be honest about the evidence though: this is theoretical, based on pharmacology and animal data, not solid human studies. Interestingly, the effect mostly showed up when berberine was taken about 2 hours before metformin, not together. Before adding phellodendron, just run it by your pharmacist or doctor so they can keep an eye on your numbers.
