Mirtazapine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP2D6.
Phellodendron contains berberine. In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, phellodendron might also inhibit CYP2D6.

Your body partly relies on an enzyme called CYP2D6 to clear mirtazapine out of your system. The berberine in phellodendron may slow that enzyme down, so in theory mirtazapine could hang around a little longer and build up. Since mirtazapine is active as-is (not a prodrug that needs switching on), this would tend to strengthen its effects.

Realistically that might mean more drowsiness, daytime grogginess, increased appetite, or weight gain. Keep in mind this caution comes mostly from lab studies, so it's a theoretical concern rather than something well proven in people. Don't stop or adjust anything on your own. Just give your pharmacist or doctor a heads-up before pairing these two.

Here's the gist: your body uses a liver enzyme called CYP3A4 to help clear mirtazapine. The berberine in phellodendron may slow that enzyme down a bit. If it does, mirtazapine could hang around longer than usual, which might nudge its effects and side effects upward. With mirtazapine, that usually means more drowsiness, grogginess, increased appetite, weight gain, or dizziness.

I want to be honest about how strong this concern is. It's mostly theoretical, based on lab work rather than real people taking these two together, so a dramatic problem isn't expected. Still, if you're considering phellodendron alongside Remeron, run it past your pharmacist or doctor first so they can keep an eye on how you feel.

What Is Mirtazapine Used For?

Brand name: Remeron

Mirtazapine tablets, mirtazapine orally disintegrating tablets, Remeron, and RemeronsолTab are all indicated for the treatment of major depressive disorder (MDD) in adults. Major depressive disorder involves a persistently low or sad mood most days, often accompanied by loss of interest, sleep changes, fatigue, and difficulty concentrating.

The effectiveness of mirtazapine tablets in maintaining a response for up to 40 weeks after an initial treatment period has also been demonstrated. Patients should be reassessed periodically to determine whether continued treatment is needed.
